Registration Instructions
Registration for either camp takes place on a first-come, first-served basis. To register, a complete application must be received no later than Wednesday June 20, 2012. The complete application must be mailed to Bright Math Camp Inc., 6 Jeremiah Place, Ottawa, K2H 8L8, and must consist of the following:
a completed camp registration form (Camp #1 and/or Camp #2);
a signed waiver of responsibility;
a signed photograph consent;
a signed acknowledgement of allergy policy; and
a cheque in the amount of $650 made payable to "Bright Math Camp Inc." (no post-dated cheques; an NSF cheque will result in an administrative charge of $50).
Upon receipt, your application will be reviewed, and acceptance into a camp may be confirmed by telephone or email. Registration may only be cancelled in writing up to June 20, 2012, but a cancellation fee of $50 will apply. There will be no refund for cancellation after June 20, 2012.
